Post-Adoption Support Services

We believe every child deserves the love, safety, and unconditional commitment of a family and the opportunity to develop as a healthy and resilient young adult. Our Pathways to Permanency team is passionate about supporting adoptive families, resource families, birth parents, and adopted children.

Adoption is a lifelong journey and we are dedicated to building a welcoming community where adoptive families can thrive. We aim to empower and uplift families with accessible, tailored resources designed to meet your unique needs.

Post-Adoption Support Services include:

• Case management services
• Trainings (in-person and virtual)
• Monthly support groups
• 24/7 Crisis support and safety planning
• Referrals and linkages to community resources
• Support with understanding the Adoption Assistance Program (AAP) and filling out county/state forms
• Therapeutic referrals
• Individualized support regarding adoption-related topics

Eligibility Requirements:

Families and/or Youth must meet one of the following Permanency and Geographical requirements:

Permanency Requirements:

• Pre-Adoptive families served by child welfare within 3 months of finalizing, OR

• Post Adoptive families with youth under the age of 26, AND

• Former Wendy’s Wonderful Kids client/family (from any state)

Geographical requirements:

• PALS: Reside within Sacramento County, or within a two-hour radius of the Sacramento Office
• PASS: Reside within Sacramento County, exceptions can be made with contractor approval
